1. Implanting a Hairline That Is Too Low
This is the biggest “sin” in hair transplantation and is almost always unforgivable. While a high hairline can be lowered later, there is no way to raise a hairline that is implanted too low. This error often occurs in younger patients wanting to replicate their peers’ hairlines. Surgeons must resist patient pressure to create an unnaturally low hairline, as this can result in an aggressive, unnatural look. A conservative design is essential, especially in younger patients who are more likely to require additional grafts in the future.

2. Implanting a Hairline That Is Too High

In reaction to the fear of a low hairline, some surgeons design the hairline too high, resulting in an unattractive framing of the face. A red flag is when patients dislike the preoperative hairline design. A balanced approach is important—sometimes the patient knows what suits them best. A hairline placed too high can accentuate hair loss and draw unwanted attention.

3. Implanting an Overly Rounded Hairline

Some surgeons apply a generic oval hairline shape to all patients, which neglects the individual’s ethnic and anthropomorphic characteristics. For example, a Slavic patient with a straight, flat forehead requires a straighter hairline than an African American, who typically has a rounded hairline. The so-called “cereal bowl” shape is unnatural and should be avoided

4. Implanting an Excessively Straight Hairline

A straight hairline appears naturally mainly in adolescents or people with broad, flat faces (e.g., Slavic descent). Inexperienced surgeons sometimes make this mistake, especially with patients who have receded temporoparietal hairlines. The shape of the forehead strongly influences hairline design. People with vertical foreheads (e.g., Asians) usually need lower hairlines, while those with sloped foreheads look better with slightly higher hairlines. The hairline angles should reflect the patient’s age, conveying maturity and fitting the inevitable progression of hair loss.

5. Implanting a V-Shaped Hairline 

 A V-shaped hairline is extremely rare in nature and tends to draw negative attention, often giving a threatening appearance. This design is sometimes seen in older transplants and is famously associated with a well-known Greek singer. It should not be confused with the “widow’s peak,” which is common and often desirable, as it creates the illusion of a lower hairline with minimal grafts.
